The sequence for the Risk Assessment and required documentation

The risk assessment include:

A. Risk Analysis
B. Risk weighting. Risk Analysis

1) Determination of the limits of the machine
2) Identification of hazards
3) Risk estimate Risk weight

The risk analysis provides the necessary information for the risk weighting, which in turn allows you to assess whether it is necessary to reduce the risk.
These evaluations must be based on a qualitative evaluation or, when appropriate, quantitative risk associated with the hazards present on the machine.
A quantitative method may be appropriate when available data. However, the application of a quantitative method is limited by the data that you have and/or limited resources of who conducts the risk assessment.

Therefore, in many applications, you can make only a qualitative risk assessment.

How to document Risk Assessment The Risk Assessment must be documented by 

Documentation showing the procedure that was followed and the results that were obtained.
This includes, where applicable, documentation of:

a) the machine to which it was carried out the risk assessment (for example, specifications, limits, intended use);
b) all relevant assumptions that have been made (loads, strengths, safety factors, etc.).
c) the hazards and hazardous situations identified and hazards considered in the risk assessment;
d) the information on which it is based risk assessment (see section 5.2):
1) the data used and their sources (historical accidents, experience gained from risk reduction applied to similar machines, etc.),
2) the uncertainty associated with the data used and its influence on the risk assessment;
e) the objectives of risk reduction to be achieved through measures of protection;
f) protective measures implemented to eliminate identified hazards or to reduce risk;
g) residual risks associated with the machine;
h) the outcome of the risk assessment (see Figure 1);
i) all forms completed during the risk assessment.

You should indicate the standards or other specifications used to identify protective measures mentioned in f) above.
